This packet contains requirements and forms for new developments that are requesting municipal sewer service. Please print, complete and return to the Authority.
Please complete and return to the LHMA office along with the tap in fee of $2600 which includes tap in and inspection. This is the APPLICATION only. After review and determination of available hookup, the permit will be issued. If application is denied permit fee will be returned to the applicant. Homeowner is responsible for excavation from the home to the tap-in site which is not included in this fee.
